About the Mobile GP Program at Ozanam House

Mobile GP is an innovative service in Victoria dedicated to empowering vulnerable older individuals to live independently. We recognise that maintaining independence requires more than just medical care—it requires a collaborative approach that integrates health and social services. Our program brings together a multidisciplinary team, including GP Telehealth, Nurse (Patient Advocate), Geriatrician/General Physician, Physiotherapist, Home Care Manager, and Mental Health Social Worker, all working together to deliver personalised support tailored to each individual’s needs.
How We Work
The Mobile GP program operates in close partnership with VincentCare Ozanam House, a leader in providing accommodation and support services to those experiencing homelessness. This collaboration allows us to extend our reach and impact, particularly among older individuals who are homeless or at risk of homelessness.
We see eligible patients at Ozanam House, where our team delivers comprehensive care on-site. The Care Manager assesses patient needs, coordinates care, and sets up essential supports, including home care packages for eligible individuals. The Social Integrator, who also serves as the Project Manager, plays a pivotal role by collaborating closely with community groups, key workers, and case managers to address the social determinants of health and ensure that each patient’s broader well-being is prioritised. The GP provides ongoing medical oversight through telehealth, supporting the efforts of the Care Manager and Social Integrator.
The Nurse (Patient Advocate) bridges the gap between patients and the healthcare team, ensuring that care plans are smoothly implemented and advocating for each patient’s needs. Regular contributions from the Geriatrician, Physiotherapist, and Mental Health Social Worker provide a holistic approach to each patient’s health and wellbeing.
We also involve caregivers and other support personnel, including those regularly engaging with patients at Ozanam House, who become crucial "patient advocates." This integrated care model fosters strong relationships, communication, and trust, creating a cohesive support network around each older person to enhance their health and independence.
Our Mission
Founded by doctors passionate about accessible healthcare, Mobile GP began as an initiative to connect patients who could not attend medical clinics with compassionate GPs. Through years of experience, we learned that the most effective way to achieve this is by leveraging a team-based approach and telehealth technology to maintain strong, ongoing connections with our patients.
Who We Serve
Mobile GP currently serves the City of Maribyrnong, City of Melbourne, and City of Yarra. Our service is available to individuals aged 50 and over (or 35 and over for Aboriginal and Torres Strait Islander people) who are at risk of developing or currently have a chronic disease. Government-funded home care packages are available for those aged 65 and over.
We prioritise support for vulnerable populations, including LGBTIQ+ individuals, culturally and linguistically diverse (CALD) communities, Aboriginal and Torres Strait Islander people, and individuals with disabilities.
Our Values
At its core, Mobile GP is a social initiative that promotes equality and self-determination. By integrating healthcare, home care and social services, we aim to address the often unmet needs of marginalised community members, creating a model of care that prioritises the whole person and supports them on their journey to independent living.
How Mobile GP integrates with home care

Many people who require the services of a Mobile GP may also benefit from a Government funded Home Care package. Integrating health care using Mobile GP, with the services provided through a Home Care package, can lead to the best outcome to support an elderly person live independently at home. We are an approved provider of Government funded Home Care packages through our sister organisation, Mark & Sylvie's Home Care.

Certified B Corporation
In 2019, Atticus Health became a certified B Corporation, where B stands for Benefit Corporation. That means we balance purpose and profit and are committed to delivering on three levels:
1. Social Performance. That's where Mobile GP comes in. We see this as our key social project. The Mobile GP project is our unique contribution aimed at changing the social fabric of life in Australia in a truly positive and meaningful way.
2. Environmental Performance. We care for and seek to conserve the natural environment. Our clinic in Jindivick Victoria, "Atticus at Jack's", is located on 2 acres of land which is home to a very special garden, including a large collection of native and exotic trees. At our other sites at both Hastings and Highett Victoria, we have likewise planted many large native trees. We have also converted to using solar energy wherever possible at our sites. Atticus Health supports the revegetation of Australia with native plants and is a member of the Austrailan Native Plants Society, Mornington Peninsula District club - https://apsvic.org.au/aps-mornington-peninsula/.
3. Financial Performance. We aim to stay strong. As an organisation, we presently employ over 100 people. All involved have the purpose of delivering outstanding medical products and services firmly in mind. It is this culture of excellence which keeps us growing.
