About
Mobile GP is a service set up in Victoria to help enable vulnerable older persons live independently at home. This is achieved by the Mobile GP team comprising Care Manager, Social Integrator and GP. The Care Manager and Social Integrator visit a patient at home, and are able to set up various supports including a home care package, if the older person is eligible. The Social Integrator in particular, considers factors relating to the social determinants of health. The GP initially visits the client at home, and after that, involves themselves via telehealth, supporting the work of the Care Manager and Social Integrator. Any other carers who may be regularly visiting the older person at home, are also relied on to contribute and in this way, become "patient advocates". The overall proposition is that there is a real team, growing relationships, communication and trust, supporting the older person, integrating health care and social services. In this way, all involved are empowered.
Started by doctors, Mobile GP had it's roots in the mission to increase access to a regular caring GP for patients who cannot get to a medical clinic. Over the years, we learned that the best way to do this is to operate as a team, and leverage telehealth to keep that connection going.
We presently offer the service to the following local council areas:
Mobile GP is available for people who are aged 50 (or over 35 for Aboriginal and Torres Strait Islander people) who are either at risk of acquiring a chronic disease or currently have a chronic disease. Note that government funded home care packages are only available for people aged over 65 (see later).
Particular groups of older persons, who may be more vulnerable than others include:
Mobile GP at its heart represents a social exchange promoting the values of equality and self determination. It seeks to fill a need for often marginalised sections of our community.

How Mobile GP integrates with home care
Many people who require the services of a Mobile GP may also benefit from a Government funded Home Care package. Integrating health care using Mobile GP, with the services provided through a Home Care package, can lead to the best outcome to support an elderly person live independently at home. We are an approved provider of Government funded Home Care packages through our sister organisation, Mark & Sylvie's Home Care.

Certified B Corporation
In 2019, Atticus Health became a certified B Corporation, where B stands for Benefit Corporation. That means we balance purpose and profit and are committed to delivering on three levels:
1. Social Performance. That's where Mobile GP comes in. We see this as our key social project. The Mobile GP project is our unique contribution aimed at changing the social fabric of life in Australia in a truly positive and meaningful way.
2. Environmental Performance. We care for and seek to conserve the natural environment. Our clinic in Jindivick Victoria, "Atticus at Jack's", is located on 2 acres of land which is home to a very special garden, including a large collection of native and exotic trees. At our other sites at both Hastings and Highett Victoria, we have likewise planted many large native trees. We have also converted to using solar energy wherever possible at our sites. Atticus Health supports the revegetation of Australia with native plants and is a member of the Austrailan Native Plants Society, Mornington Peninsula District club - https://apsvic.org.au/aps-mornington-peninsula/.
3. Financial Performance. We aim to stay strong. As an organisation, we presently employ over 100 people. All involved have the purpose of delivering outstanding medical products and services firmly in mind. It is this culture of excellence which keeps us growing.
